Description: Hospital for Special Care is a private, not-for-profit rehabilitation and chronic disease hospital. The social worker is part of an interdisciplinary team working to enhance the abilities of every patient.  Every patient has individual goals created by the team, patient and family. The social worker provides counseling and support to patients and families regarding emotional changes and assists in planning the patient's discharge.

Description: HRA is a community action agency located in New Britain.  It administers many social services and program sites.  

Someplace Special provides assistance to individuals and families who are infected with and/or affected by HIV and AIDS.  Assistance with transportation, respite care, drug treatment, childcare, homemaking, companion support is available. Activities conducted include

 educational workshops, a resource library, support groups, art therapy, massage therapy, social and recreational events.

Supportive Housing Program for People with AIDS HRA administers rental subsidies and provide support services to families living with HIV/AIDS throughout New Britain.  Case management services are offered in an attempt to maintain safe, affordable housing and improve the quality  of life for families faced with the challenge of HIV/AIDS.

Young Latinas Program provides concrete knowledge and methods for prevention of HIV/AIDS to young Latinas ages 13 to 29 years and their partners. Services include outreach, mentoring, group meetings and educational presentations

Description: Its mission is to end the devastating effects of Multiple Sclerosis, a neurological disease. Coordinate outreach to CT health professionals with a goal of building client referral base.  Identify mental health and rehabilitation professionals to receive referral survey, follow-up to assure survey completion.  Assist with with outcome measures for programs and services to assure quality service and identify client needs.  Develop implement and tabulate evaluation surveys for key chapter programs.
