Description: Hospital for Special Care is a private, not-for-profit rehabilitation and chronic disease hospital. The social worker is part of an interdisciplinary team working to enhance the abilities of every patient.  Every patient has individual goals created by the team, patient and family. The social worker provides counseling and support to patients and families regarding emotional changes and assists in planning the patient's discharge.

 Inpatient Rehabilitation Multi-levels of care are available to offer therapy to patients with amputations, neurological conditions, orthopedic problems, multiple trauma, arthritis and severe muscle weakness from prolonged complicated hospitalizations.  Head Injury Program; Pulmonary Rehabilitation; Stroke Rehabilitation  For more information: www.hfsc.org

Description:  To educate, inform and support parents of children with disabilities for the purpose of improving their children's education and quality of life.  AFCAMP is a parent run organization.  All board members, staff and volunteers are parents and caregivers of children with disabilities.  AFCAMP provides parent to parent support; education & referral; training for families & professionals; information on services and disability laws; Advocacy; monthly support meetings

Description: Its mission is to end the devastating effects of Multiple Sclerosis, a neurological disease. Coordinate outreach to CT health professionals with a goal of building client referral base.  Identify mental health and rehabilitation professionals to receive referral survey, follow-up to assure survey completion.  Assist with with outcome measures for programs and services to assure quality service and identify client needs.  Develop implement and tabulate evaluation surveys for key chapter programs.
